Key takeaway

GHRP-2 (pralmorelin) is a synthetic ghrelin-mimic peptide that prompts the pituitary to release growth hormone, sold in the wellness market for muscle, recovery, and appetite. Its only genuine validated use is as a diagnostic test of growth hormone capacity (approved in Japan), not as a treatment, and there are no controlled trials supporting the marketed wellness benefits. As of 2026 it is not FDA-approved and is banned in sport.

What is GHRP-2?

GHRP-2, with the generic name pralmorelin, is a synthetic six-amino acid peptide that imitates ghrelin to stimulate growth hormone release. It is sold through wellness and "research peptide" channels and promoted for muscle gain, recovery, and increased appetite. Unlike most unapproved peptides, it has a narrow legitimate role: it is approved in Japan as a diagnostic agent to test the pituitary's growth hormone capacity.

How does it work?

GHRP-2 activates the growth hormone secretagogue receptor (the ghrelin receptor) on the pituitary. Activating it has three reported effects:

  • It triggers a pulse of growth hormone, and growth hormone pulses raise IGF-1 downstream.
  • It tends to modestly raise other hormones such as cortisol and prolactin.
  • Like other ghrelin mimics, it can stimulate appetite1.

The marketed muscle and recovery claims are extrapolated from this hormone effect rather than demonstrated. This describes the proposed mechanism, separate from proven outcomes.

Is it FDA-approved?

No. GHRP-2 is not approved by the FDA; its only regulatory approval is as a diagnostic agent in Japan. The table below sets out its status with each regulator.

RegulatorStatus
FDANot approved, with no approved product in the U.S., and not approved for performance or anti-aging
U.S. compounding rulesTreated as not eligible for routine 503A compounding
JapanApproved as a diagnostic agent
WADAGrowth hormone-releasing peptides including GHRP-2 are prohibited in sport at all times under the WADA list

What does the research show?

The validated human data concern diagnosis, not treatment. According to PubMed, GHRP-2 is used to probe pituitary growth hormone reserve, as in a study2 of the growth hormone response, which is a diagnostic application rather than a wellness-outcome trial. There are no controlled human trials showing that GHRP-2 improves muscle, fat, recovery, or aging. The marketed uses rest on its ability to raise growth hormone, not on demonstrated benefit.
